The ESRD Treatment Choices Learning Collaborative (ETCLC) has developed a system to track the nature and progress of organization improvement initiatives using change concepts developed in the collaborative change package.  The “Progress Tracker” is a signaling system intended to surface organizations generating significant progress in developing effective practices in support of the collaboratives 3 aims.  Likewise, the tracker can help identify those organizations or QI teams that might be struggling and in need of assistance. The “Progress Tracker” features a webform that is completed on a monthly basis.  The input for this form is created after a discussion of the status of each of the progress milestones identified on the form.  This discussion is expected to be held with the appropriate members of the organizational team and regional leads, OPOs or the QI Team Coach prior to completion of the form.
